Initial Comments:
An off-site, state licensure follow-up survey conducted on December 20 and December 28, 2021 found that Personal Home Healthcare and Hospice had corrected the deficiencies cited under 42 CFR, Part 418, Subparts A, C and D: Conditions of Participation: Hospice Care. The deficiencies were cited as a result of a state re-licensure survey completed on October 28, 2021.
